To save time, you may print and complete these forms, and bring them with you to your first visit. To view these PDF forms, you’ll need the freely available Adobe Reader software installed on your computer.

Keep in mind, you may need assistance with filling out forms if your dominant/writing hand is affected. If paperwork is not completed prior to your first visit, please arrive at least 20 minutes prior to your appointment time to complete necessary paperwork. Please bring a photo ID and your insurance card to every visit.

You are required to have a referral for Hand Therapy. Ask your doctor for a referral prior to scheduling an appointment or filling out new patient paperwork.
