Navigating the administratively complex world of Workers’ Compensation can be challenging for clinicians who do not specialize in the field of Occupational Medicine. Our team’s extensive experience in Occupational Medicine facilitates authorization of care to optimize recovery and minimize time lost from work. Our standardized processes supported by electronic medical record-keeping results in less administrative work for the employee, employer, and carrier. Effective communication with each of these stakeholders increases transparency and minimizes unnecessary delays.

Our team is committed to providing medically appropriate and compassionate care to the injured worker with the expectation that both the employee and employer will do their best to support recovery and a return to full duty as soon as safely possible. Treatment decisions are based on a thorough history including mechanism of injury and prior health, a focused physical exam, and evidence-based medical literature.

On-site we have a hand therapy department, digital x-ray, and an open MRI. A variety of surgical specialists work on-site and are typically available to consult emergently on complex cases.

Our Philosophy

At FORM Ortho, our goal is to support injured workers and their employers with continuously excellent case management. One of our strategies is to remain the Primary Treating Physician (PTP) for the duration of the claim or until it no longer benefits recovery or case management. When we consult with a specialist they are assigned the role of Secondary Treating Physician; primary responsibility for care is not transferred to the specialist. This permits FORM Ortho providers to continue case management including the return to work decisions.

We communicate directly with the specialist and incorporate their reports into our own. We continue to see the injured worker for follow-up visits to coordinate overall care and disability management optimally. This permits the consultant to focus on their area of specialization, including surgery, while we continue to provide input on our area of strength: supporting progressive recovery while providing employers and adjusters with timely, structured, and comprehensive progress reports. On rare occasions, a particularly complex case will medically warrant a transfer of care and we work with the adjuster and the specialist to ensure this is accomplished efficiently.

Our ability to provide timely and comprehensive care is facilitated by being located in the Farwell Medical Building which is equipped and staffed for the optimal treatment of musculoskeletal conditions.
